FORMS
Youth Medical Authorization Form:
The Youth Medical Authorization Form authorizes the designated representative of Bethany Lutheran Church to act on behalf of the parent or legal guardian for medical treatment of a minor under their care while participating in Bethany Youth events. This form is to be updated as required.
Youth Account Transactions Form:
The Youth Account Transactions Form is used by the youth or parent to request funds for church related activities from the youth's savings account. These accounts are administered by the church with the monies earned by the youth through church fund raisers.
Volunteer Background Check Form:
Anyone wishing to work with Youth in our congregation must have a current Volunteer form on file (a new form should be filled out each year). New volunteers must have form completed at least 2 weeks prior to the event they'd like to help with. It can be found here. After completed, please turn in to the Church office.

SCHOLARSHIPS
Bethany Lutheran Church has been richly blessed with an excellent professional staff. In gratitude to God for such blessings, and understanding the need to continue to provide quality professional workers here and in the Lutheran Church-Missouri Synod at large, a scholarship program has been established by the congregation. The Bethany Scholarship Fund, administered by the Board of Christian Education, is maintained to encourage young men and women of our parish to enter full time service within our church. Scholarships are therefore made available to any student from Bethany who is attending our synodical colleges or seminaries or Valparaiso University with the intent of entering full time church work.
Scholarships will be given on a graduated basis to any student from the congregation who had submitted an application and declaration of intent to enter the full time ministry with our congregation.
Understanding between the congregation and the student:
- Bethany Lutheran Church encourages all students preparing for a full time church work profession to make application by July 23rd for the funds for the forth coming academic year. One half of the annual scholarship will be presented to the student before the fall semester. The second half of the scholarship will be given to the student in January of the following calendar year.
- We as a congregation hope that the student would contribute to the life and ministry of Bethany as they are available during vacation periods.
- We as a congregation, especially through our ministerial staff, commit ourselves to providing opportunities for these young men and women to "field test" some of their training which might be suggested through their academic program or as an aid to our congregation.
- We as a congregation promise to continue to encourage each student through prayer, counsel, and moral support that he or she might serve our Lord and His church in this very high vocation.
Several members of Bethany are pursuing full time church work with assistance from the Bethany Scholarship Fund.
